In the event that you wish to cancel your membership with UHC Healthcare and the UHC Healthcare Preventive Health Plan Membership Card issued to you, you shall be required to intimate the same to UHC Healthcare by calling UHC Healthcare help desk or by sending an email to email@example.com before 14 (fourteen) days of your planned last day use of the UHC Healthcare Preventive Health Plan Membership’s Welcome Pack. You shall further be required to return the UHC Healthcare Preventive Health Plan’s Membership’s Welcome Pack, with all documents and other items provided to you along with the Welcome Pack (“UHC Healthcare Preventive Health Plan Welcome Pack”) by mailing the aforesaid to the UHC Healthcare Cancellation Department at C-2 SEC-12 NOIDA UTTAR PRADESH
On receipt of such intimation and the UHC Healthcare Preventive Health Plan’s Welcome Pack, UHC Healthcare shall verify the details of your membership and the request for cancellation placed by you. On UHC Healthcare satisfaction that your request is in accordance with these Terms, UHC Healthcare shall carry out the following (collectively, “Cancellation”):
Discontinue the UHC Healthcare Preventive Health Plan Membership Card issued to you and your membership with UHC Healthcare with immediate effect; and issue a refund of the amount paid by you as fees towards purchase of such UHC Healthcare Preventive Healthcheck Membership Plan (“Membership Fees”).
On Cancellation as aforesaid, UHC Healthcare shall intimate you of the fact of such Cancellation and shall inform you of the method of refund of your Membership Fees. Such Cancellation shall be subject to the following terms:
Cancellations to new order shall be considered only if (i) a valid request is made in this regard within 72hours of purchase order generated by sending email with all valid supporting document showing that customer himself/herself cancelling the order. As UHC Healthcare process the product PO only after confirming with customer by call and once order confirmed we deliver the product and send the delivery report to the customer after then no cancellation will be accepted.
No Cancellation shall be carried out if you have purchased the UHC Healthcare Preventive Health Membership Plan under any celebratory / festive offer, or any special / discounted offer of any kind whatsoever.
No Cancellation shall be carried out if you have purchased the UHC Healthcare Preventive Health Membership Plan under any interest free easy monthly installment (EMI) option offered by UHC Healthcare.
No Cancellation shall be carried out if you have in any way availed any of the services/benefits available under the UHC Healthcare Preventive Health Plan’s Membership prior to confirmation of Cancellation by UHC Healthcare.
In the event a request for Cancellation is received by UHC Healthcare after dispatch of the UHC Healthcare Preventive Health Plan’s Welcome Pack to you, UHC Healthcare shall deduct an amount of Rs. 3,000/- (Rupees three thousand only) or 40% (Four per cent) of the Membership Fees, whichever is higher, from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation process.
In the event that (i) the entire UHC Healthcare Preventive Health Plan’s Welcome Pack is not received by UHC Healthcare within 14 (fourteen) days of receipt of a valid request for Cancellation by UHC Healthcare, or (ii) a request for Cancellation is received by UHC Healthcare after a welcome call has been made by UHC Healthcare to you wherein you have confirmed your membership, UHC Healthcare shall not be entitled for any refund or compensation to you.
All Cancellations approved by UHC Healthcare shall ordinarily be processed within 21 (twenty one) days of receipt by UHC Healthcare of the UHC Healthcare Preventive Health check Plan’s Welcome Pack. Credit of the Membership Fees to your bank account / credit card account upon Cancellation and refund by UHC Healthcare shall be as per policies and timelines followed by the banks involved and UHC Healthcare shall not be responsible for any delays in this regard.
In case of any fraudulent transactions on http://uhchealthcare.in/ UHC Healthcare will reimburse only 50% of the amount two payee through cash in case transaction is less than 1 lac or check otherwise, Rest 50% will be deducted as service charge and other costs to company. The 50% refundable amount will be transfer to the customer through relevant mode of payment within 70 working days. In case UHC Healthcare has not been notified within 15 days of transaction no refund shall we made to any clame
LOST CARD: In case of loss of the UHC Healthcare Preventive Health Plan Membership Card, a duplicate membership card may be issued by UHC Healthcare. You shall be required to pay nominal charges of Rs. 150 + applicable taxes/- (Rupee one hundred fifty only) for issuance of such duplicate card. The tenure of your membership will remain as specified in the initial UHC Healthcare Preventive Health Plan Membership Card.

PROVISION OF SERVICES BY MEDICAL PROVIDERS: The provision of Medical Services provided by Medical Providers to you shall, in addition to as otherwise stated, be subject to the following:
The Medical Provider will offer the Subsidised Fees on Medical Services offered by the Medical Provider directly within the premises of such UHC Healthcare medical network partner. The Subsidised rate on medical services shall not be applicable to any further service required from any other doctor/hospital, whether referred by the Medical Provider or otherwise. All medical services, tests, medical fields and specializations may not be under the UHC Healthcare Preventive Health Membership Plan. Further, the Subsidised Fees may not be offered.
